You can download all the required health forms and specific requirements for new and returning students below. If you are looking for a form and can’t find it here, contact us and let us know.

Health Forms:
Here is the 2020 Health Form Information Checklist. This will guide you through which forms are needed for which grades.
All health forms are due by August 28.
In order to attend the first day of school, all students regardless of grade, must have a copy of their current immunization record on file in the nurses office.
All health forms should be submitted through Magnus Health, which you can access directly or through the AFS Portal, a secure system for submitting required health information to AFS. If you have questions about required health forms, please contact Susan Harris, School Nurse. You can print out the required health forms linked below on this page.

Health Forms for New Students
Tuberculosis Assessment Form
Required for all new students prior to the start of the school year.
DownloadPhysical Examination Form
Required for all new students from Preschool to grade 4.
For grades 5 - 12 the submission of a current Athletics (PIAA) Physical Form meets the requirement.
Medication Permission
Required for any over-the-counter or prescription medications to be administered to your student during school hours.
DownloadHealth Appraisal Form
Required for all new students, preschool to grade 12, prior to the start of the school year.
This form is completed by the parent or guardian and does not require a doctor's signature.
Athletics Physical (PIAA) Form
For 2020-2021, please contact Athletic Director Jeff Bond or School Nurse Susan Harris for updates on the requirements and the school athletics program.

Required for returning students in grades K, 7 and 11.
